Some of What You'll Do:
The SUDP is responsible for providing chemical dependency diagnosis and treatment and behavioral health services in a professional, positive, consumer-friendly, and recovery-focused manner. The SUDP is a core member of a collaborative care team that includes the patient's primary care provider; a consulting psychiatric provider; other behavioral health staff; and other primary care support and care coordination and behavioral health staff. The SUDP is also responsible for coordinating referrals to clinically indicated services both within and outside the primary care clinic (e.g., social services, mental health specialty care, inpatient substance abuse treatment, housing, employment, peer support).
