Position Overview
This
position provides clinically appropriate outreach and engagement services to
individuals with identified or suspected Substance use disorder needs. This SUDP will function as an adjunct
intervention specialist with the Cowlitz County Sherriff's Office. The
Substance Use Disorder Professional is a core member of a collaborative care
team that includes a consulting mental health provider, and other primary care
support and care coordination and behavioral health staff. Engagements are provided to address a broad
array of substance use disorder needs, including coordinating referrals to
clinically indicated services. Services
are provided with a context of multi-agency collaboration with an emphasis on
effective, recovery-based outcomes.
Natural and formal supports are actively involved in treatment as
appropriate. The First Responder SUDP
will be a member of the Columbia Wellness Outpatient Department.
Summary of the Essential Duties and
Responsibilities
- Access individuals of all ages to determine diagnoses,
needs, goals, strengths, supports, & culturally relevant issues impacting
behavioral health needs
- Identify and develop an appropriate course of care
delivery that includes immediate referral to crisis intervention /
stabilization services and/or withdrawal management services as deemed
medically necessary and appropriate
- Engage with community service providers to ensure
appropriate assessments and treatment courses are identified and pursued
- Continually re-assess
existing contacts to identify engagement and referral needs
- Provide case-management services as necessary
- Consult with relevant community agencies, the
coordination of multi-agency services, and participation in multi-agency
support teams on behalf of consumers
- Provide appropriate substance use disorder training to members of the Sheriff's
Department as needed
- Participate in
regular supervision
- Display a professional image to the community
- Ability to be flexible and manage schedule appropriately
to address workload demands and trends
- Ability to motivate staff and develop a working team
around individual cases
- Effective time management including but not limited to
planning ahead where possible and be punctual in person and work performed
- Build strong working relationships and a positive work
environment
- Be receptive to feedback, willing to learn, and embrace
continuous improvement.
- Complete all documentation, reporting, and other
paperwork per agency standards
- Attend all appropriate staff, organizational, and
external meetings (as assigned)
- Work efficiently both independently and as part of a
team.
- Other duties as assigned.
Minimum Qualifications:
- Must have an associate degree in human services or related field from an
approved school per WAC 246-811-030; or
- 1-2 years' experience
in substance use disorder treatment and services; 4 years preferred
- Substance Use
Disorder Professional certification/licensure with the WA Department of Health
- Must have working
knowledge of ASAM Patient Placement Criteria as well as DSM 5
- Must be knowledgeable
of Washington Administration Codes and the Revised Code of Washington, to ensure
compliance with all requirements.
- Able to maintain
confidentiality in accordance with HIPAA, 42 CFR, 45 CFR, and RCW 70.96A.
- Experience working
with clients who have co-occurring mental health, substance abuse, and physical
health problems.
- Experience with evidence-based
counseling techniques (e.g. MRT, Seeking Safety, 12-Step, etc.)
Preferred
Experience: Proficiency providing services
to a variety of populations, which may include people experiencing
homelessness, underserved or marginalized populations, formally incarcerated
individuals, Veterans, people with disabilities (both physical and emotional /
behavioral), and individuals who identify as LGBTQ.
Salary: $23.88/hr.
