Position Overview
This position provides care
coordination, comprehensive care management, health promotion and
engagement. Natural and formal supports
are actively involved. The Care
Coordinator actively engages high risk clients employing strategies to reduce unnecessary
hospitalizations of Youth eligible for Medicaid in the state of Washington and
at risk of out of home placement and involved in multiple systems.
Summary of the Essential Duties and
Responsibilities
- Comprehensive care
management and coordination.
- Assist in development
of crisis plans, practice and rehearse them as part of the transition phase.
- Provide 24 hour crisis support to clients
enrolled in WISe several days out of each month as scheduled.
- Facilitate Child and
Family Team (CFT) meetings and guide the team process.
- Be the central point
of communication for members of the CFT.
- Encourage each CFT
member to identify their priority concerns, work proactively to minimize areas
of potential conflict, and acknowledge the mandates of others involved in
child-serving systems.
- Utilize
consensus-building techniques to meet the needs of the youth and family.
- Establish and sustain
an effective team culture by inviting CFT members to propose, discuss, and
accept ground rules for working together.
- Increase the “natural
supports” in CFT membership and the youth/family's integration into their
community.
- Work with Youth
Partner and/or Family Partner to identify family support, peer support or other
system and community resources that can assist the youth and family with
exercising their voice in the CFT process if needed.
- Prepare for CFT
meetings.
- Contact CFT members
who are unable to attend meetings, in advance, to elicit their input.
- Ensure all CFT
members receive appropriate documents.
- Maintain team focus
on scope of work for the WISe team and progress/movement toward transition.
- Be sensitive to the
needs of team members when working in rural areas where getting members
together physically may be challenging. Creative in establishing a team that
may meet via phone or through teleconferencing.
- Ensure respect for
the input and needs of the youth when forming the team.
- Acknowledge and
celebrate successes and transitions.
- Referral to community
and social support services.
- The oversight of
those therapeutic and case-management services, which may be provided by
counselors, counselor-techs, and counselor assistants for those clients
assigned to the Care Coordinator.
- Participates in
individual supervision from Clinical Supervisor.
- Effective use of
technology and software including but not limited to: computers, phones, email,
EMR, etc.
- Ability to work with
difficult cases and vulnerable populations.
- Be helpful,
respectful, approachable and team-oriented.
- Effective time
management including but not limited to: planning ahead where possible and be
punctual in person and work performed.
- Build strong working
relationships and a positive work environment.
- Be receptive to
feedback, willing to learn, and embrace continuous improvement.
- Complete all
documentation, reporting, and other paperwork per agency standards.
- Attend all
appropriate staff, organizational, and external meetings (as assigned).
- Work efficiently both
independently and as part of a team.
- Ability to be
flexible and manage urgent and crisis situations effectively.
- Ability to motivate
staff and develop a working team around individual cases.
- Other duties as
assigned.
Minimum Qualifications:
- Master's degree with at least one year of experience working
in social services; or
- Bachelor's degree with at least two years of experience
working in social services; or
- Is an individual with lived experience who has already
received their certified peer counselor certificate or can obtain it upon hire.
- Upon hire: professional registration or certification by the
Licensing Department of the State of Washington as a counselor.
- CANS certification or obtain within 30 days of hire.
Preferred Qualifications:
- State of Washington
mental health specialty in, child, DD, older adult, and/or ethnic populations
preferred.
Salary:
Starting at $18.33
This position is a part of a union (OPEIU Local 11)
