Some of What You'll Do:

This position provides care coordination, comprehensive care management, health promotion and engagement.  Natural and formal supports are actively involved.  The Care Coordinator actively engages high risk clients employing strategies to reduce unnecessary hospitalizations of Youth eligible for Medicaid in the state of Washington and at risk of out of home placement and involved in multiple systems.

What We Require:

·  Master's degree with at least one year of experience working in social services; or

·  Bachelor's degree with at least two years of experience working in social services; 

·  Upon hire: professional registration or certification by the Licensing Department of the State of Washington as a counselor.

·  CANS certification or obtain within 30 days of hire.

Preferred Qualifications:

  • State of Washington mental health specialty in, child, DD, older adult, and/or ethnic populations preferred.

Salary: $ 17.83 per hour

This position is a part of a union (OPEIU Local 11)
